Please take notice that a public hearing will be held before the Planning Commission of the City of Black River Falls on Wednesday, June 7, 2017 at 5:30 P.M. at City Hall in the City of Black River Falls to hear the petition of Black River Memorial Hospital to obtain a conditional use permit for Black River Memorial Hospital to construct a professional office building on the following described land and premises:
Northeast ¼ of the Northwest ¼ of Section 15, Township 21 North Range 4 West, City of Black River Falls, Jackson County, Wisconsin.

The conditional use permit would allow Black River Memorial Hospital to construct a professional office building on the premises for the purpose of medical records, coding, transcription, release of information, customer service, insurance billing, credit and collection service, financial assistance, and other patient-related business matters.
